Form can be printed, filled out and sent along with membership fee to the address
on the bottom. Join us and start going on some great trail rides.
I (we), the undersigned, do hereby release,
discharge, and hold harmless, the Cave Creek Saddle Club, its members and officers, their assigns and
successors from any and all claims arising out of accidents, or incidents, during
any trail ride or social function held by the Club.

Dues and Membership
___ I’m delighted that the dues have been
reduced this year and I understand that although CCSC is an
ASHA member that
I can join ASHA individually - AT A REDUCED COST (see their membership
application). Membership in ASHA is no longer a part of your
club membership.

parties we’ll have.
